Select the topic question "I want to request a copy of my immunization record from NJIIS." This will open the help center request form (ticket). Click on link “Request a copy of NJIIS Immunization Record". This will open the IMM-46 form which you need to complete. You can attach the form directly to this ticket. Tomorrow morning and Wednesday morning, September 8, trucks will collect bulk materials from Orange residents such as furniture and other household items. Please neatly place all acceptable items at curbside on your lawn for collection. There will be a dumpster for additional bulk items at the Old Police Station, located at 595 Lincoln Avenue for West and South ward residents There will also be a dumpster at the Public Works yard located at 24 South Center Street for East and North Ward residents.
